Post Acute Care

Country Care provides Post Acute Care Services. These services are designed to meet those care needs your may require, generally for short term and funded on discharge from Hospital.

These services/tasks are identified and requested by your Health Care Professionals to assist in your transition when leaving Hospital and returning home.
