Your Care Team

Once we have your Care Plan we can put together your Care Team.

You will be involved in the selection of your attendant carers. We will support you to interview potential attendants and will provide them with any initial training they require.

You can expect your Attendant carers to:

• Be reliable & on-time
• Be courteous to you, your family and friends
• Complete the duties asked of them
• To respect your privacy, dignity and confidentiality.
• Be willing to work flexible hours

In turn it is essential that you treat your Attendant carers with courtesy and be clear with them about the assistance you need and when you require it.

If you want to change the duties your Attendant carers are doing, talk with them about the changes and how they feel about them. If you reach an agreement you are both happy with - contact the Office so we can amend your Care Plan. If you have difficulties contact us and we will assist.

Once your care plan is in place and your team is established we provide ongoing care management to ensure your plan meets your needs.
