Your Care Plan

Our first step is to meet and discuss with you, your family and your Health Care Professionals to develop a Care Plan.

We will talk with you about the amount of attendant care you require and what type of duties you would like your attendant carers to assist you with.

Each person will have a different set of tasks they need their attendant carers to do. This is why an individualised Care Plan is written for each person.

The Care Plan sets out details such as:

• the number of hours of attendant care you have
• the sorts of duties these attendants will perform
• any additional details required to provide your care.

Once we have your care plan we can look at selecting your Care Team.
