Transitional Care

During your transition from institutional, hospitalisation or rehabilitation to independant living at home, special Transitional Care is required while your Care Team is established.

We will arrange a Transitional Care Team to assist you when you first move back home. This skilled team of experienced attendant carers are especially trained to provide interim care support to people returning to remote rural areas.

This means discharge from hospital need not be delayed while your Care Team is being established and the anxiety of returning home can be reduced.

This team can be maintained while Country Care assists you with trialing and training your permanent team of attendant carers.
