Post Acute Care

Country Care provides Post Acute Care Services .  These services are designed to meet those care needs your may require, generally for short term and funded on discharge from Hospital.

These services/tasks are identified and requested by your Health Care Professionals to assist in your transition when leaving Hospital and returning home.
