Ongoing Care Management

Once your Care Plan is complete and your Care Team is established we provide ongoing Care Management to ensure your plan is meeting your needs.

Needs change from time to time, and as your needs change we may need to adjust your plan.  To do this reviews of your Care needs are conducted with you and your Support Workers.

Some people with disabilities like additional assistance with arranging and co-ordinating the services they need. We can work closely with your local Case Managers/Support Workers of other services or can assist you with finding and using other services
